I think they were playing tag, he tagged the duckling in panel 4, then the Mom duck and Duckling began to chase him and he surrendered, he was just putting his paw up for them to tag him

Uhm, cats actually tend to kill birds.
Now, cats tend not to be doctors either, but the thing is this: the key premise of your awesome comic is to elaborate on how the doctor’s work is counteracted by the cat’s natural instincts (mousey toys, licking paws, etc.) (or rather what the reader perceives as natural instincts). But this also means that departing from the cat’s instincts – it would try to kill the birds – deals a serious blow to the storytelling premise.
Who cares? It’s cute.
